Reproductive Health Clinics Experiences Survey

This survey is open to the public, pregnant people and health care workers to report misleading and deceptive conduct by Crisis Pregnancy Centers (also known as pregnancy resource centers, pregnancy care clinics, and fake women's health centers).
Examples include but are not limited to:
- Misrepresenting services offered
- Misrepresenting the danger of abortion
- Delaying reproductive health services through duplicative tests
- Hidden fees
If you are eligible to participate, you will complete a short consent form before taking the survey.
The survey will take about 5 minutes. No personal information is required to be provided, but you may choose to if you would like to be put in touch with further resources.
Who can take the survey?
Anyone can use this form to report their own experience or on behalf of another individual.
Who receives the information I share in the survey?
Information you share on the survey will only be viewed and analyzed by research staff. Before you take the survey, you will see detailed information about how we will use your report.

Yes. The survey does not ask for identifying information unless you would like us to contact you.
