4D Recovery is a nonprofit organization based in Portland, Oregon, that provides recovery support services for youth and young adults ages 14-35, including substance use treatment, mental health services, peer mentoring, housing, and community centers. You can learn more by visiting our website. We're currently seeking a part-time (5-20hrs/week) Prescriber to join our clinical treatment team.
Experience & Qualifications Required or Preferred:
- This position requires at least a Masters’ Degree and additional training as a Nurse Practitioner, Physicians’ Assistant, or higher levels of medical training.
- This position typically requires reliable transportation or a valid driver’s license for the state of residency and a driving record permitting coverage under the corporate liability policy. If driving a personal vehicle for work purposes, must also have and maintain personal auto liability coverage. All employees driving for work purposes, regardless of whether driving a personal or agency vehicle, must complete the agency’s driving approval process before they drive for work.
- Ability to use most features of Google Workspace and Microsoft Suites at a high level.
- Ability to create detailed reports as requested.
- Ability to work independently and as a team player.
- Excellent communication, trauma-informed approach and conflict resolution skills.
- Well-developed documentation skills and a strong commitment to quality management; ability to write accurate and concise reports while meeting tight deadlines.
- Must be dependable, flexible and punctual, and exhibit strong time management skills.
- Able to clearly articulate information to people of varying cultural, linguistic, and educational backgrounds. Able to communicate clearly, concisely, and effectively both orally and in writing.
- Able to problem solve and develop creative approaches.
Responsibilities:
- Client interactions
- Conduct thorough initial evaluations including medical history, substance use history, psychological status, and social needs.
- Educating patients and their families about addiction, recovery process, treatment options, and healthy lifestyle choices.
- Regularly assessing patient progress through clinical interviews, urine drug screens, and other monitoring tools.
- Diagnosis and treatment planning
- Identifying, diagnosing, or confirming mental health conditions and substance use disorders, creating personalized treatment plans with appropriate interventions and goals.
- Prescribing and monitoring medications, including medication for opioid use disorder (MOUD) like buprenorphine, sublocade, naltrexone, etc. as per treatment protocols.
- Prescribing medication for mental health concerns like depression, anxiety, trauma, and PTSD within the context of substance use treatment.
- Case management
- Collaborating with other treatment center staff like therapists, counselors, and medical providers to ensure coordinated care.
- Coordinating care with other healthcare providers, social service agencies, and community support systems to address patients’ holistic needs.
- Documentation
- Maintaining accurate and detailed medical records, including treatment plans, progress notes, and medication administration documentation.
- Other duties as assigned by the Clinical Director.

Our supportive and collegial urgent care team specialize in same-day acute care, acting as an emergency diversion program. We have full-time and part-time positions with clinic hours and Monday-Friday from 10am - 8, and Saturday-Sunday from 8:00am-5pm.
