3: Verify your identity

• Confirm your email and/or phone.

4: Check your phone or email for the code

• Verifying the code completes the MyChart two-step verification process for your device. 

After you complete MyChart two-step verification

You only need to set up two-step verification in MyChart once on a device.

• If you sign in from a different device, you will need to repeat two-step verification by entering a code sent to your email or phone. 
• If you signed up for a new MyChart account, you can complete MyChart two-step verification the next time you sign in.  

Recover your MyChart username and password

Follow these steps if you forget your 博彩网站 MyChart username or password. 

To recover your username

Verify your first name, last name, date of birth and ZIP code.  

• After you click Submit, you will get an email from mychart@ohsu.edu. 
• The email subject will say “MyChart Verification code." 

To reset your password

Verify your username, date of birth and ZIP code. 

• After you click Submit, you will get a one-time code sent as text message or email. 
• Enter the 6-digit code into MyChart to continue.  

Create a new username or password

• Enter and verify your new username or password.
